What Factors May Cause Diabetic Nephropathy

2019-02-18 09:11

Diabetic Nephropathy,FactorsDiabetes therapists tell us that diabetic nephropathy has no obvious symptoms in the early stage. With the prolongation of the course of the disease, persistent proteinuria, edema, hypertension, glomerular filtration rate decreased, and finally caused uremia. Then the causes of Diabetic Nephropathy are:

1. Hyperglycemia:

The occurrence of diabetic nephropathy is closely related to hyperglycemia. Poor control of blood sugar can accelerate the occurrence and development of diabetic nephropathy, and good control of blood sugar can significantly delay its development. The proliferation of mesangial cells, the increase of extracellular matrix, the expansion of mesangium and the thickening of glomerular basement membrane were caused by the increase of hyperglycemia and glycation end products.

2. Hypertension:

It is not directly related to the occurrence of diabetic nephropathy, but elevated blood pressure during the course of hypertension or microalbuminuria can accelerate the progress of diabetic nephropathy and deteriorate renal function, and aggravate the excretion of urinary albumin.

3. Abnormal renal hemodynamics:

(1) In hyperglycemia, glomerular hyperperfusion, hyperfiltration and increased pressure across the capillary wall cause mesangial cells to expand, epithelial cells to fuse and produce dense droplets, and glomerular epithelial cells to fall off from the basement membrane.

(2) In the case of increased pressure, protein filtration increases, and can also deposit in the mesangium and glomerular basement membrane, promote matrix proliferation, form a vicious cycle, and cause nodular and diffuse glomerulosclerosis.

(3) Glomerular basement membrane type IV collagen messenger ribonucleic acid increased, the basement membrane became thicker, eventually forming a diffuse mesangial nodular lesion, glomerulosclerosis.

4. Hereditary factors:

Differences in the expression and regulation of glucose transporter-1 menu in mesangial cells among individuals with diabetes mellitus may be one of the factors that predispose some patients to kidney damage. Moreover, the occurrence of diabetic nephropathy also shows the phenomenon of family aggregation. In some diabetic patients with family history of hypertension, the incidence of diabetic nephropathy is significantly higher than that of patients without family history of hypertension. In addition, the incidence of diabetic nephropathy varies among different races. All these indicate that the occurrence of diabetic nephropathy is related to genetic factors.

Diabetes treatment experts told diabetic patients with symptoms of nephropathy, should be treated as early as possible, do a good job of daily care, to avoid aggravation of diabetic nephropathy incentives, to prevent diabetic nephropathy to endanger the lives of patients in the later period.
