The Comparison Comments Between Hemodialysis And Peritoneal Dialysis

Patients with end-stage nephropathy usually need to receive peritoneal dialysis (PD) or hemodialysis (HD) treatment. Although PD has a lower medical cost compared with HD, its clinical application is relatively limited.Whether there is a difference in the survival of patients with end-stage renal disease who receive PD and HD is unclear.Wong B, from the university of Calgary in Canada, conducted a retrospective cohort study to determine whether there were differences in the survival of HD and PD patients.The results were published in AJKD.

The researchers selected 2032 ESRD patients suitable for both HD and PD treatment from 7 dialysis centers, and compared the survival of HD(n=1579) and PD(n=453) patients.The average period of follow-up was 520 days.

During follow-up, it was found that there was no significant difference in all-cause mortality between HD and PD in patients aged 65 with >.In patients younger than 65, PD patients had a 40% lower risk of death 3 years before dialysis than HD patients.

Excluding patients who were not suitable for PD treatment, further analysis showed that there was no significant difference in the mortality of patients receiving HD and PD treatment regardless of age (0.47 vs. 0.38).

"At present, there are many studies on the survival of HD or PD patients," the researchers said. "although most studies show no significant difference in mortality between HD and PD patients, this study is aimed at patients who are suitable for both HD and PD, and the study deviation is relatively small."When patients with end-stage kidney disease cannot choose between two dialysis regimens, the results of this study can be used to guide doctor-patient decisions and have clinical significance.

Dr Simonetta Carla Genovesi, from Italy, commented: 'the subject of this study was a patient who was suitable for both HD and PD therapy, so as to minimize the deviation of the study.After adjusting for confounders, the researchers found that there was no significant difference in mortality between HD patients and PD patients, which was beneficial for patients (no PD contraindication) to choose their own dialysis methods freely.

However, it should be noted that compared with other industrialized countries, the national PD treatment rate in Canada is 22%, about twice as high as that in the United States and Italy (10%).That is to say, the national PD promotion rate in Canada is higher than that in other countries, while the results of this study may not be meaningful for those countries that cannot carry out PD promotion."
