Uremia Stage May Not Need Dialysis, What Is The Key

2019-02-20 13:38

Not Need Dialysis,Uremia Stage,KeyWhen the serum Creatinine of Kidney Failure patients reaches about 700, doctors often recommend that patients undergo Dialysis for life. However, some patients do not need dialysis at this time.

The renal function of these patients remained stable for a long time within 3 stages (filtration rate above 30, creatinine below 442), and did not fluctuate much in peacetime. In a check-up, suddenly found more than 600 serum creatinine, or even more than 700, kidney disease to stage 5 (uremia, filtration rate below 15, creatinine above 707), so the local hospital doctors may ask you to carry out hemodialysis as soon as possible.

At this time, we should know that you are in chronic and acute, whether you need to take dialysis or not.

The progress of Chronic Kidney Disease is slow, glomerular sclerosis and necrosis, tubulointerstitial fibrosis is generally a slow process. Due to all kinds of reasons, which can be relieved.

Patients must inform the doctor of their previous renal function status and serum creatinine values, fully communicate, and find out the acute progression factors that make serum creatinine rise significantly. If acute factors can be relieved within a month, renal function may recover and serum creatinine may decrease.

What causes a sharp rise in serum creatinine?

1. Infection

This is the most common acute factor of nephropathy.

There are three most common infections in nephrotic patients:

Respiratory tract infection: including colds, tonsillitis, bronchitis and pneumonia, may have cough, sputum, fever, sore throat, chest tightness, chest pain and other symptoms.

Urinary tract infection: There may be symptoms such as frequent urination, urgency, pain in urine or low back pain.

Digestive tract infections: such as gastroenteritis, may have abdominal pain, nausea, vomiting, fever, loss of appetite, fecal incontinence and other symptoms.

Other infections include skin infections.

2. Drug injury

Antipyretic and analgesic drugs, stomach-protecting drugs that inhibit gastric acid, tacrolimus, cyclosporine, some antibiotics and nephrotoxic traditional Chinese medicines may cause acute damage of renal tubulointerstitium.

3. Water loss

In the case of excessive dehydration such as sweating, fever, vomiting and diarrhea, the kidney may be damaged due to insufficient blood flow, resulting in a rapid and substantial increase in serum creatinine.

4. Hypertension

Such patients usually have high blood pressure. It may be that antihypertensive drugs do not control blood pressure. It may also occur that hypertension itself does not know, so that hypertension damages the kidney.

In addition, moderate to severe anemia, fatigue, staying up late, high-protein diet, heavy drinking, disease activity period (lupus kidney is more common) and other factors may also lead to a sudden sharp increase in serum creatinine.

These conditions should be treated and recovered within one month as soon as possible. If long-term dialysis is started at this time and acute kidney injury lasts for more than a month, the acute disease will turn to chronic disease and can no longer be saved.

Moreover, dialysis can accelerate the loss of residual renal function. so if you are in acute creatinine increased, that you may do not need dialysis.
